Description RN Manager - Care Management Social Work at
Providence Portland Medical Center in Portland, Oregon. This is a
Full-Time (1.0 FTE), Day Shift position. We offer an annual

structure. The Care Management (CM) Manager is accountable for the
direction and management of the inpatient Care Management
Department. This position provides leadership and oversight for the
clinical care management model, staff of approximately 24-31 FTE,
budget of approximately $2.4-2.9 M, and operations of the care
management department for the facility. S/he promotes team
functioning, advances the clinical care management model and
improves coordination of patient care in collaboration with all
other disciplines. Works with the Regional Director CM, PHSOR
leaders, clinical professionals, health plans and Coordinated Care
Organizations, and community and government agencies as appropriate
to establish effective discharge planning mechanisms and develop
new models for transitioning patients back into home and community
settings. The CM Manager effectively interacts with Medical Staff,
facility executives/leaders, Utilization Management, Finance and
continuum and community partners. At the facility level, the CM
Manager coordinates and provides oversight of use of the Patient
Assistance Fund programs. This position will operate as a strategic
partner to other Oregon PH&S facilities in meeting the Triple

Degree in Nursing Oregon Registered Nurse License upon hire 2 years
of Supervisory experience, including program planning, performance
management, staff development and evaluation. 1 year of Care
Management experience. 3 years of Relevant experience in one or
more of the following areas: Social Services. Utilization
Review/Quality Management. Case Management. Preferred
