Job Description:
Description Schedule: Full Time, Days, Monday- Friday 8am-5pm
Location: Hybrid (onsite at Halsey Building in East Portland and/or
remote) The Home Health Access RN is part of the team that helps
link Providence Home Health current patients, current patient's
family members and caregivers, physicians, physician's office
staff, service inquiries and referral sources. This position is
primarily nursing advice and phone triage (which may include doctor
orders, care coordination within the teams). The Home Health Access
RN coordinates with the care teams related to nursing needs of
patients admitted into Home Health. Working in Home Health provides

Experience using electronic medical record software. Upon hire:
Oregon Registered Nurse License Washington Registered Nurse License
within 30 days of hire Upon hire: In compliance with the Providence
Health and Services-OR Human Resources policy 302 Background
Checks, a criminal background check is required for this position,
based on assigned duties. Preferred Qualifications: Bachelor's
Degree Nursing Nursing Triage Call Center experience. Med/Surg

Posted are the minimum and the maximum wage rates on the wage range
for this position. The successful candidate's placement on the wage
range for this position will be determined based upon relevant job
experience and other applicable factors. These amounts are the base
pay range; additional compensation may be available for this role,
such as shift differentials, standby/on-call, overtime, premiums,
