Job Description:
Kaiser Permanente Georgia seeks a dedicated RN Case Manager to
enhance the healthcare journey of our members. This role involves
detailed assessment, planning, and coordinated management of
patient care across multiple settings to achieve optimal health,
access to care, and appropriate utilization of resources. This
position fosters a collaborative, patient-centered approach to
care, reflecting our commitment to providing exceptional healthcare
services where care and coverage are seamlessly integrated. Conduct
comprehensive assessments of patient health needs and preferences.
Develop, implement, and regularly update individualized care plans.
Facilitate communication and resource sharing between patients,
families, and healthcare providers. Monitor patient progress and
outcomes, adjusting care plans as necessary. Ensure compliance with
healthcare regulations and clinical standards. Advocate for
patients’ needs and optimal outcomes at all healthcare touch
points. Educate patients and families about healthcare processes,
conditions, and management. Strong clinical skills in a healthcare
setting Excellent interpersonal and communication skills Proficient
in healthcare management systems and record keeping Ability to work
effectively in a team-oriented environment Strong organizational
and problem-solving skills Medical Insurance Dental Insurance
Vision Insurance Life Insurance 401k Current Registered Nurse (RN)
license in the state of Georgia. Certification in Case Management
(CCM) preferred.
