Legacy Health is seeking a passionate and skilled Diabetes Educator to join our dynamic team across our network of hospitals in Oregon and Washington. This role is essential in providing specialized knowledge and support to patients with diabetes, guiding them through the complexities of their condition and contributing to a holistic approach to health and wellness. The successful candidate will collaborate with healthcare professionals to tailor educational and therapeutic interventions that empower patients to manage their health effectively. Assess patient education needs, developing individualized diabetes management plans. Provide detailed explanations and demonstrations on how to properly use diabetes monitoring equipment and insulin pumps. Organize and conduct group sessions and workshops to educate patients and families about diabetes management. Stay updated with the latest trends and research in diabetes care to provide evidence-based guidance. Collaborate with multidisciplinary teams to enhance patient care and outcomes. Document patient progress and communicate with healthcare providers about any necessary adjustments in treatment plans. Excellent communication and interpersonal skills Proven ability to tailor educational content to diverse audiences Strong organizational skills and ability to manage multiple tasks Deep knowledge of diabetes pathology, management techniques, and education strategies Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Certified Diabetes Educator (CDE) or eligibility to obtain certification within one year of employment Active Registered Nurse (RN) or Registered Dietitian (RD) license preferred
