Description Occupational Therapist- Providence Home Services
Oregon Schedule: Full Time/ Days Supports Damascus, Sandy, Boring
and Welches Manages and provides skilled occupational therapy to
patients and clients with a wide variety of diagnoses and
disabilities in a home setting in accordance with a physician
directed plan of treatment, taking into account individual patients

more! Required qualifications: - Bachelor's Degree From an
accredited Occupational Therapy Program Or; - Master's Degree From
an accredited Occupational Therapy Program Or; - Ph.D. From an
accredited Occupational Therapy Program. - Within 30 days of hire:
National Provider BLS - American Heart Association. - Upon hire:
Oregon Occupational Therapist License. - Washington Occupational
Therapist License (Vendor Managed) - if practicing within this
