Description Social Worker (LCSW or CSWA) - Hospice of the Gorge
at Providence Hospice Of The Gorge in Hood River The Licensed
Clinical Social Worker (LCSW) independently provides comprehensive
biopsychosocial/spiritual assessment, diagnosis and/or
treatment/interventions/advocacy of patients and their support
systems. The LCSW incorporates concepts and theories associated
with life span development, neurobiology, and behavioral change.
The LCSW will collaborate with the patient and support system to
develop and implement evidence-informed person-centered care plans
across the care continuum in collaboration with the
Inter/Multi-Disciplinary Team (IDT). The LCSW shall perform duties
in a manner that respects patient self-determination, that reduces
health disparities/barriers to care, and adheres to the NASW code
of ethics, Standards of Practice (SOP), and mission. Required
qualifications for this position includes:
- Master's degree in Social Work from a Council of Social Work
Education (CSWE) accredited school of social work, or from a school
in candidacy status by the CSWE
- Licensed Clinical Social Worker (LCSW or CSWA) with the state
of Oregon
- Licensure as an LICSW in the state of Washington required as
needed based on work location(s)
- Two (2) years relevant Social Work experience. May include
Clinical Social Worker Associate (CSWA) experience
- American Heart Association (AHA) Basic Life Support for Health
Care Provider completion within one month of hire
- Valid driver's license
- In compliance with the Providence Health & Services-OR Human
Resources policy #302 Background Checks, a criminal background
check is required for this position, based on assigned duties
- Employees providing services in the State of Washington must
pass the Washington Access to Criminal History (WATCH) background
check
- It is required that an application to become an Agency
Affiliated Counselor through the Washington State Department of
Health must be submitted within 7 days of hire Preferred
qualifications for this position include:
- Social work experience
- 2 years Clinical work experience in Hospice or Palliative
Care
- 2 years working independently with vulnerable populations in a
